How Do I Change My Name on File? - Lived and Legal Name

NOTE: All legal and livid name changes will not affect your email address. ONLY your displayed name. 

For current and prior students:

The Registrar’s Office must be contacted for a legal name change. The Registrar’s office can be contacted by Phone at 708-524-6774 or by email at registrar@dom.edu. You can also visit the Registrar's Office in Lewis Hall 129. Office hours are Monday - Thursday: 8:30am-6:30pm and Friday 8:30am-4:30pm.

Once approved, please forward us the email confirmation so that we can update your displayed name and Star Card. Our email address is supportcenter@dom.edu. 

For Faculty and Staff:

In order to change your legal name on your accounts, you must first contact the Office of People and Culture and change it officially through them. For more information on how to do this, contact OPC by email at opc@dom.edu. You can also visit the Office of People and Culture in Lewis Hall 234. Office Hours are Monday-Friday, 8:30 a.m. – 4:30 p.m.

For All Users with a MyDU Login:

To do a lived name change on just Dominican portals, there is a form that can be filled out in MyDU. Once you have logged into MyDU, there is a column on the left hand side of the screen that brings you to forms you will need to fill out. This form goes to the SSE for approval, then shortly after you will be contact by IT to pick up your Star Card with the new name.
