I Recently Changed My Name. How Do I Get My Email, Accounts, and Records Changed to Reflect My New Name?

For current and prior students:

The Registrar’s Office must be contacted for an official name change. The Registrar’s office can be contacted by Phone at 708-524-6774 or by email at registrar@dom.edu. You can also visit the Registrar's Office in Lewis Hall 115. Office hours are Monday - Thursday: 8:30am-6:30pm and Friday 8:30am-4:30pm.

For Faculty and Staff:

In order to change your name on your accounts, you must first contact Human Resources and change it officially through them. For more information on how to do this, contact HR by phone at 708-524-6655 or by email at hr@dom.edu. You can also visit the Office of Human Resources in the Lewis Annex, 1st Floor. Office Hours are Monday-Friday, 8:30 a.m. – 4:30 p.m.

For All:

To do a name change on just Dominican portals, there is a form that can be filled out in Mydu. Once you have logged into MyDu, there is a column on the left hand side of the screen that brings you to forms you will need to fill out to get them changed.
